เงื่อนไขการรับบริการ
1. ขอสงวนสิทธิ์การให้บริการสำหรับท่านที่นำ E-Coupon มาแสดงเท่านั้น
2. ในกรณีที่ไม่นำ E-Coupon มาแสดง ท่านต้อง ชำระค่ามัดจำ ตามราคาชุดตรวจ
3. E-Coupon นี้ไม่สามารถแลกเปลี่ยนเป็นเงินสดได้
4. E-Coupon นี้สำหรับการตรวจตามรายการที่ระบุเท่านั้น ไม่สามารถเปลี่ยนแปลงได้
5. สามารถใช้สำหรับผู้รับบริการผู้ป่วยนอก (OPD) และผู้รับบริการที่ช่วยเหลือตัวเองได้เท่านั้น
6. ชุดตรวจสุขภาพ ไม่รวม ค่าแพทย์ในกรณีเจาะจงแพทย์ผู้เชี่ยวชาญในการตรวจสุขภาพและตรวจทางรังสี
7. ชุดตรวจสุขภาพ ไม่รวม ค่ารักษาหรือค่ายาเพิ่มเติมในกรณีที่แพทย์สั่ง
8. กรุณานัดหมายล่วงหน้า และงดอาหาร ก่อนเข้ารับบริการ 10-12 ชั่วโมง (จิบน้ำเปล่าได้)

10. สามารถรับบริการที่ ศูนย์เวชศาสตร์อายุรวัฒน์ ชั้น 1 อาคาร R โรงพยาบาลกรุงเทพ (ซ.ศูนย์วิจัย) ทุกวัน 7:00 – 15:00 น.

วันหมดอายุ E-Coupon 
1. E-Coupon มีอายุการใช้งานภายใน 180 วันนับจากวันที่สั่งซื้อ
2. หาก E-Coupon หมดอายุ รพ.ขอสงวนสิทธิ์การเข้าใช้บริการ และ E-Coupon ไม่สามารถขยายระยะเวลาในการใช้บริการหรือคืนเป็นเงินได้
3. ชุดตรวจสุขภาพราคานี้ สามารถสั่งซื้อได้ถึง 31 ธันวาคม พ.ศ. 2567 เท่านั้น

หมายเหตุ
1. กรณีลูกค้ามีการขอยกเลิกคำสั่งซื้อหลังสิ้นสุด Shopee Guarantee โดยไม่ได้มีสาเหตุมาจากทางโรงพยาบาล โรงพยาบาลขอสงวนสิทธิ์ในการหักค่าดำเนินการ 10% จากราคาสุทธิในใบเสร็จ
2. ยอดเงินที่แสดงใบเสร็จเป็นยอดเงินที่ได้รับจากการชำระเงินจริงไม่รวมโปรโมชั่นส่วนลด

1. Service is reserved exclusively for customers who present the E-Coupon.
2. In case the E-Coupon is not presented a deposit must be paid according to the package price.
3. This E-Coupon cannot be exchanged for cash.
4. E-Coupon is only applied to specific items as mentioned and changes cannot be made.
5. These packages are for outpatients who are physically dependent. 
6. These packages exclude doctor fees in case additional specialists are required.
7. These packages exclude treatment fees or medications prescribed by the doctors if any. 
8. Please make an appointment in advance and refrain from eating 10 - 12 hours before the service  (drinking water is allowed).For further information or to make an appointment please contact the Health Design Center at 02-3103284 02-7551591 (operating hours: 07:00-15:00) or the Contact Center at 1719.
9. Services can be received at the Health Design Center 5th Floor Building R Bangkok Hospital (Soi Soonvijai) 
every day from 7:00 to 15:00.

Expiration Date of E-Coupon:
1. The E-Coupon is valid for 180 days from the date of purchase.
2. If the E-Coupon expires the hospital reserves the right to refuse service and the E-Coupon cannot be extended or refunded.
3. This health check-up package can be ordered until December 31 2024

Note:
1. In case customers request cancellation of the order after the Shopee Guarantee period without valid reasons from the hospital the hospital reserves the right to deduct a 10% processing fee from the net amount on the receipt.
2. The receipt amount displayed is the actual payment received excluding any promotional discounts.
